Anatomy of the Iliotibial (IT) Band

The iliotibial band (IT) is a thick band of connective tissues in the lateral side of the thigh, which arises from the fascia in the gluteal muscles. Some authors consider it as a reinforcement of the fascia lata itself. 

In the knee, it is responsible for maintaining stability on the outer side of the joint both in extension and in partial flexion, and is therefore used constantly during walking and running. It is one of the main supports of the knee against gravity. A bursa at the top of the knee assists in the smooth movement of the IT band over the lateral femoral condyle during flexion or extension.

The constant motion of running, cycling or walking long distances results from excessive friction between the bursa and iliotibial band as it passes over the bony prominence on the outer side of the knee. This causes excessive irritation and inflammation. With prolonged irritation, the band tends to tighten more due to repeated rubbing, which causes pain along the outside of the knee joint. If the symptoms are not treated, the knee can become swollen and more painful and in extreme cases, scarring of the bursa is expected, which further increases the pain and restricts range of motion.

 

Symptoms of ITB Syndrome

  • Sharp or burning pain along the outer side of the knee at the joint line over the bony prominence, radiating either proximally or distally
  • Pain usually develops gradually over time, becoming progressively worse.
  • Pain is made worse by running, particularly downhill
  • Inflammation and swelling on the lateral side of the knee
  • Tenderness over and around the bony prominence
  • Popping sensation on the lateral aspect of the knee with movement

 

Treatment of IT Band Syndrome

  • Inflammatory stage, (RICE) – rest, ice, compression and elevation are the first approach to treatment
  • Physical therapy which includes modalities, stretching and strengthening exercises for further rehabilitation
  • Activity modification by avoiding aggravating factors

 

Treatment with Massage

Sports massage is very beneficial in loosening the iliotibial band. While treatments should be aimed mostly in the ITB itself, the surrounding muscles, especially the tensor fascia lata and gluteals, may also be tight. Massaging these sites is recommended to release the tension at the source.

Massage Procedures Using Recovapro Massage Gun for ITBS

  • Along the tensor fascia latae extending to the gluteals on the lateral thigh, apply short gliding strokes, friction and static compression using the round head attachment, adjust speed and power accordingly
  • Along the ITB, apply gliding strokes also with the round head attachment
